Medical Records If you would like to request a copy of your medical records, they can be obtained through our ShareCare portal. You will be required to provide a valid email address and a government-issued ID. Third Party Medical Records Requests: For healthcare providers, attorneys or if you are requesting the medical records for someone other than yourself and are not the legal guardian.
